Director of Social Services - Long Term Care

The Pines at Whiting has a rare opening for a Full Time Director Social Services to oversee our Skilled unit Hamilton Place, and Georgetown Assisted Living. We are a facility with an excellent survey record and are looking for someone to round out our healthcare management team.

This individual is responsible for planning, directing, coordinating, and evaluating the medically related social and emotional needs of the Resident. Responsible for organization of the Social Services department and coordinating its function with all other department and community resources. Responsible for the coordination of the admission process.

1. Plans, Directs, and organizes the social services program to meet the objectives of the health center as part of total resident care.

2. Prior to, and upon admission, meets with resident/family to assess and plan for social service needs

3. Assists om conducting facility tours

4 Counsels as needed to adequately prepare Resident and family for admission

5. Obtains and coordinates financial, medical, and other required information. Develops social history on each Resident upon admission.

6. Orients all new residents and families to new environment and assists adjustment through involvement in both individual and group programs

7. Incorporates social service needs of Residents/families as evidenced by psychological assessment with interdisciplinary plan of care, identifying and addressing medically related social and emotional needs for each resident. Actively participates in all interdisciplinary conferences,

8. Documents on a timely basis all information required by regulation and facility policy, including an initial social service assessment, on-going progress notes and discharge plan.

9. Designs and documents social services programs for both individual resident and groups.

10. Communicates and collaborates with external referral sources and internally with all staffs to assure maximum care for residents/families. Maintains a current community resources file to identify and promote support networks.

11.  Provides residents and families with intervention skills in the areas of adjustment, remotivation, and other behavioral programming; health, orientation and self-help skills and other appropriate modalities.

12. Other duties / projects as assigned by healthcare administrator

Requirements

1. Active New Jersey License as a Social Worker

2. Bachelor's Degree in Social Work Preferred. 2-3 years of supervised social work experience in a health care setting (Preferred LTC). or similar professional qualifications may be substituted as allowed by regulations. This social service position is regulated by both state and federal guidelines.

3. Two years experience in case work desirable

4. Ability to assess Residents and effectively communicate with all residents including those with sensory deprivation.

5. Working knowledge of applicable federal and state regulations

Certified Medical Assistant - Independent Living

We are seeking a full time Certified Medication Aide to provide care and administer evening medications to our independent living residents. You will provide basic care needs of the residents under the direction of the Clinic Nurse and Clinic Manager. Medication administration will be administered under the direct delegation of the clinic nurse and clinic manager.

1. Responds to emergency calls from independent living residents

2. Observes residents for verification of medication consumption and possible adverse reactions

3. Observes residents and reports any issues to the clinic nurse and/or clinic manager

4. As requested, assists residents in all activities of daily living but not limited to bathing, dressing, grooming, nutritional needs, oral hygiene, personal laundry tasks and housekeeping

5. Other duties as assigned by the clinic nurse and/or clinic manager.

Must be compassionate and understanding in order to meet the needs of the residents.Must have an active CMA license from the State of New Jersey. Must be willing to work 2-10 Monday through Friday.

Seeking that compassionate person that loves the geriatric community to represent the leading assisted living for memory impairment You will be leading activities on our dementia unit. This is part time weekend position working 15 hours a week.

The ideal candidate should have excellent interpersonal skills and have the passion to lead and work with our residents. Candidates with previous dementia experience is a plus/
